Why physicians look

Practice medicine the way you want to.

Physicians rarely come to us because they want to run a different kind of business. They come because they want more time with patients, and fewer things standing between the two of them.

  • Unhurried visits

Appointment length set by what the visit actually needs.

  • Time you control

A schedule shaped around care rather than volume targets.

  • Relationships that last

Knowing your patients over years, not single episodes.

Whether a different payment model gets you there depends on your specialty, your market, and your patients. That is the question worth working through carefully.

By Doctors For Doctors

You have options for how you practice.

Changing the way your practice is paid is a significant decision. OptOutMD helps you understand the options, identify the questions that matter, and approach the transition with your patients and practice in mind.

These are distinct decisions. Your starting point depends on your current practice and your goals.

Planning the transition

A thoughtful transition considers the whole practice.

These are the areas worth working through before a change reaches your schedule, your team, or your patients.

  1. Practice readiness

Your goals, current payment arrangements, team, and capacity for change.

  2. Financial planning

Pricing assumptions, operating costs, patient demand, and transition cash flow.

  3. Requirements and responsibilities

Questions for your legal, billing, and other professional advisers.

  4. Patient communication

Clear explanations of payment changes, choices, and next steps.

Our approach

From questions to a clearer plan.

Begin with a conversation about where you are today and what you want to change. From there, we can discuss the education and guidance appropriate to your goals.

Independence should keep patients at the center.

A different payment model changes the conversation with patients. Thoughtful planning includes clear information about costs, how the practice will work, and the choices available to them.

We built the kind of practice we wanted to work in. OptOutMD was created to help other physicians do the same.

The TruOrtho story

Built from a real independent practice.

OptOutMD did not start as a theory. The model was shaped by the work of building and operating TruOrtho, an independent orthopaedic practice.

Through TruOrtho, Dr. Amanda Marshall-Rodriguez has dealt firsthand with the questions physicians run into when they want more control over their time and operations, less unnecessary administrative burden, a better experience for their patients, and a practice that is sustainable to run.

The systems, decisions, and practical lessons that came out of that work shaped what OptOutMD offers other physicians today: a clearer path toward independent practice, informed by having actually done it.

Common questions

Answers before you reach out.

  • Who does OptOutMD work with?

We work with physicians and practice owners who are thinking about how their practice is paid. That includes physicians considering Medicare opt-out, physicians weighing a move away from commercial insurance, and physicians who simply want to understand the landscape before deciding anything.

  • Is this only for orthopedic surgeons?

No. Orthopedic surgery is a founding area of focus and shapes how we think about surgical practices, but the questions involved in practice payment change are shared across many specialties.

  • Is Medicare opt-out the same as dropping commercial insurance?

No. They are two separate decisions with separate requirements.

  • What does direct-pay actually mean?

Direct-pay describes a payment arrangement in which the patient pays the practice directly rather than the practice billing an insurer for that service.
